*	GALA dinner is included in the registration fee. You can participate the 1WCO as well with 30PSM registration.

**	Accompanying persons fee includes: Opening Ceremony, Closing Ceremony, Welcome Reception, Japan Night, Gala Dinner and access to Exhibition Hall

Cancellation policy

Cancellations by March 13 (Fri.)	100% of the total will be refunded.
Cancellations by April 10 (Fri.)	50% of the total will be refunded.
Cancellations after April 10 (Fri.)	No Refund

Cancellation should be noticed to the secretariat by E-mail (psm30@c-linkage.co.jp) with the participant’s name and his/her Registration ID.

※耳鼻咽喉科専門医の単位について: 第30回ポリッツアー国際耳科会議に参加いただくと、耳鼻咽喉科専門医5単位が認定されるよう、日本耳鼻咽喉科学会へ申請いたします。
IDカードでの受付は行いませんので、学会当日受付に『学術集会参加報告票』を提出してください。
